As a recipient of federal funds from the Federal Highway Administration and the Federal Transit Administration, the Chicago Metropolitan Agency for Planning (CMAP) complies with Title VI of the Civil Rights Act of 1964 to ensure that no person shall be excluded from participation in, denied the benefits of, or subjected to discrimination under any program or activity provided by CMAP because of race, color, or national origin. The Title VI Program was adopted by CMAP’s MPO Policy Committee in June 2017. Lee en español Title VI complaint process Any person who believes they have been discriminated against on the basis of race, color, or national origin by CMAP may file a Title VI complaint by completing and submitting the agency’s complaint form. File a complaint with CMAP To file a Title VI complaint with CMAP: Download a copy of CMAP’s Title VI complaint form; request a copy by writing to CMAP at 433 West Van Buren, Suite 450, Chicago, IL 60607; or call 312-454-0400 Complete, date, and sign the complaint form Submit the complaint to CMAP no more than 180 days after the alleged incident, to:Title VI CoordinatorChicago Metropolitan Agency for Planning433 West Van Buren, Suite 450Chicago, IL 60607 Once the complaint is received, CMAP will notify the complainant with an acknowledgment letter informing whether CMAP has jurisdiction to investigate. CMAP investigates complaints that are received no more than 180 days after the alleged incident and with complete information. CMAP has 60 days to investigate the complaint. CMAP may contact the complainant if more information is needed to resolve the case. If the complainant does not contact CMAP or CMAP does not receive the additional information within 40 business days, CMAP can administratively close the case. A case can also be administratively closed if the complainant no longer wishes to pursue the case. After reviewing the complaint, CMAP will issue one of two letters to the complainant: A closure letter summarizing the allegations and stating that there was no Title VI violation, and the case will be closed, or A letter of finding which summarizes the allegations and the investigation, and explains what corrective action will occur If the complainant wishes to appeal the decision, they have 60 days after the date of the closure letter or the letter of finding to do so.
